River Towne Care and Rehabilitation Center in Columbus, GA is seeking an experienced MDS Coordinator (RN) to manage resident assessments and ensure compliance with CMS guidelines. You will coordinate MDS 3.0, OBRA and PPS assessments, and work with the IDT to develop individualized care plans, documenting timely and accurate MDS data.

The role requires an active RN license, CPR certification, and strong organizational and analytical skills. Monday–Friday 8am–4pm with no holidays or weekends.

  • Complete and submit MDS 3.0 assessments in compliance with CMS guidelines.
  • Schedule and coordinate OBRA and PPS assessments.

  • Review and verify clinical documentation to support MDS coding.
  • Initiate and update Care Area Assessments (CAAs).
  • Collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans.
  • Participate in care plan meetings with residents, families, and staff.
  • Monitor Quality Measures (QMs) and ensure accurate reporting.
  • Manage MDS assessments related to PDPM.

Join us and be part of our growing team of skilled caregivers in our newly renovated facility! River Towne Care and Rehabilitation Center, a Pyramid Healthcare facility, is in Columbus, GA. The Center provides long-term skilled nursing care and short-term post-acute rehabilitation, including a 30-bed ventilator unit. Job Summary:

The MDS Coordinator (Minimum Data Set Coordinator) is responsible for managing the resident assessment process and ensuring compliance with federal and state regulations, including Medicare/Medicaid reimbursement guidelines.

Hours:

Monday - Friday 8am-4pm; No Holidays, No Weekends

Salary:

Negotiable related to experience with MDS, Care Planning, Case Mix Index, and Quality Measures; Bonus Incentives included

Key Responsibilities
  • Complete and submit MDS 3.0 assessments in compliance with CMS guidelines.
  • Schedule and coordinate OBRA and PPS (Medicare Part A) assessments.
  • Ensure accuracy and timeliness of MDS data for all residents.
  • Review and verify clinical documentation to support MDS coding.
  • Initiate and update Care Area Assessments (CAAs).
  • Collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans.
  • Participate in care plan meetings with residents, families, and staff.
  • Ensure resident care plans reflect current status, risks, and goals.
  • Monitor for significant changes in condition and trigger timely assessments.
  • Maintain compliance with state and federal regulations regarding MDS and resident assessments.
  • Audit charts and MDS records for accuracy, completeness, and timeliness.
  • Prepare and assist during state surveys, audits, or quality reviews.
  • Monitor Quality Measures (QMs) and ensure accurate reporting.
  • Manage MDS assessments related to PDPM (Patient-Driven Payment Model).
  • Ensure proper Medicare/Medicaid reimbursement through accurate coding
  • Track ARDs (Assessment Reference Dates) to optimize reimbursement.
  • Educate nursing and interdisciplinary staff on MDS processes and documentation.
  • Provide guidance on documentation to support coding and compliance.
  • Mentor new nurses and assist with staff orientation on resident assessments.
  • Monitor residents for significant changes that may require updated assessments.
  • Review daily clinical and nursing reports to identify MDS-related triggers
  • May assist with direct care or clinical tasks within scope of license (varies by facility).
  • Maintain MDS tracking logs and assessment calendars
  • Submit data to CMS QIES / iQIES or equivalent systems.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Assist with compiling and analyzing facility performance metrics (e.g., rehospitalization rates, falls, pressure injuries).
Education Requirements
  • Must be a currently RN in good standing with the state board of nursing.
  • CPR certification required.
Qualifications:
  • Minimum of 2 years nursing experience in a long-term care or rehabilitation setting.
  • Active RN license in the practicing state.
  • Minimum of 1–2 years of long-term care experience, with MDS knowledge.
  • Knowledge of CMS regulations, RUG-IV, or PDPM
  • Proficient in MDS, electronic medical records (EMR), and documentation systems.
  • Strong organizational, analytical, and communication skills.
  • Strong leadership, organizational, and communication skills.
  • Ability to work collaboratively with interdisciplinary teams and residents' families.
  • Basic computer proficiency and documentation skills.
Work Conditions:
  • Work is performed in a skilled nursing facility setting, with frequent interaction with residents, families, staff, referral sources, and community members.
  • May require occasional evening or weekend hours based on facility needs, admissions, community outreach, or special events.
  • Must be able to wear appropriate personal protective equipment (PPE) as required for infection control purposes.
  • Must be able to maintain a professional appearance and conduct while representing the facility.
