Job summary

The Mariemont Care Center is seeking an experienced MDS Coordinator to join our nursing team. The role requires active Ohio RN licensure and extensive MDS/RAI experience, with the ability to coordinate assessments and care plans across departments.

Responsibilities include managing the RAI process, maintaining documentation for CMS/state compliance, and ensuring timely completion of MDS schedules while supporting resident care and reimbursement.

Qualifications

• Active Ohio RN license required.
• Experience with MDS processes and RAI.
• Knowledge of MDS 3.0 and care planning.
• Strong attention to detail and organizational skills.
• Ability to work independently and meet deadlines.
• Strong communication and teamwork skills.

Responsibilities

• Coordinate and complete MDS assessments accurately and on time
• Manage the Resident Assessment Instrument (RAI) process
• Develop and update individualized care plans
• Coordinate with nursing, therapy, dietary, social services, and other departments
• Participate in interdisciplinary care plan meetings
• Review clinical documentation for accuracy and compliance
• Monitor MDS schedules and deadlines
• Ensure documentation supports resident care and reimbursement
• Maintain CMS/Medicare/Medicaid and state regulatory compliance
