Job summary

River Towne Center in Columbus, GA, seeks an experienced MDS Coordinator to manage the resident assessment process and ensure compliance with federal and state regulations, including Medicare/Medicaid reimbursement guidelines. The role emphasizes MDS 3.0, PDPM, OBRA, and care planning within an integrated IDT.

Hours: Monday–Friday 8am–4pm; no holidays or weekends. RN in good standing with CPR certification required.

Qualifications

• Must be an RN in good standing with the state board.
• CPR certification required.
• Minimum of 2 years nursing experience in long-term care or rehabilitation.
• Knowledge of CMS regulations, RUG-IV, or PDPM; proficient in MDS and EMR.

Responsibilities

• Complete and submit MDS 3.0 assessments in CMS guidelines.
• Schedule and coordinate OBRA and PPS assessments.
• Ensure accuracy and timeliness of MDS data for all residents.
• Review clinical documentation to support MDS coding.
• Initiate and update Care Area Assessments (CAAs).
• Collaborate with IDT to develop individualized care plans.
• Participate in care plan meetings with residents and families.
• Monitor Quality Measures and ensure accurate reporting.
• Manage MDS assessments related to PDPM and reimbursement.
• Educate staff on MDS processes and documentation.
