Job summary

CareCore at Lima in Lima, OH is seeking a dedicated full-time MDS Coordinator to ensure compliance with federal and state MDS guidelines and manage Medicaid/Medicare RUGs/PDPM.

This on-site role requires an Ohio-licensed nurse (RN preferred), at least one year of MDS coordination experience, strong organization, and proficiency with EHR/MDS software; benefits include PTO, holidays, health insurance, CE opportunities.

Qualifications

• Licensed RN in Ohio; RN preferred.
• Minimum of 1 year of experience as an MDS Coordinator or equivalent role.
• Strong understanding of MDS 3.0 guidelines and skilled nursing facility regulations.
• Knowledge of the Medicare and Medicaid reimbursement process.

Responsibilities

• Ensure compliance with federal and state MDS guidelines.
• Manage Medicaid and Medicare RUGs/PDPM and Medicaid Case Mix.
• Meet Medicare criteria and maintain required supportive documentation.
• Collaborate with clinical and administrative teams to optimize care and documentation.
