Job summary

Transitions Healthcare Shook Home is seeking a Nurse Assessment Coordinator (RN) to manage the resident assessment process in line with Federal and State regulations. The role involves case management and leading PPS/Medicare meetings with the interdisciplinary team to ensure compliant, timely MDS submissions.

The ideal candidate holds an active PA RN license, with 2 years in institutional care and MDS 3.0 experience, plus CPR/AED certification. Competitive wages and benefits offered.

Qualifications

• Registered Nurse in good standing in Pennsylvania (PA).
• Two years' experience in institutional resident care preferred.
• MDS 3.0 knowledge required.
• CPR and AED certification current.

Responsibilities

• Develop and coordinate the resident assessment process per Federal/State rules and policy.
• Provide case management and lead PPS and Medicare meetings.
• Oversee MDS, CAA’s and Care Plans with interdisciplinary team.
• Coordinate MDS data via medical records, bedside assessment, and interviews.
• Ensure timely transmission and completion of MDS documents.
