Job summary

RBRC, a leading provider of nursing and rehabilitation services, seeks an experienced Registered Nurse Assessment Coordinator (RNAC)/Medical Records Coordinator to ensure regulatory compliance, resident-centered care, and survey readiness. This on-call role supports the facility's MDS, RAI, and care planning processes.

You will coordinate OBRA and Medicare assessments, maintain entry/re-entry records, and communicate coverage status to residents and families, collaborating with CHC case managers

Qualifications

• Current LPN/RN license required.
• Basic computer and EHR/PCC knowledge and experience required.
• Preferred RAC-CT certification.
• Experience with RMDS and care planning.
• Long term care experience preferred.
• Strong organizational and communication skills.

Responsibilities

• Coordinate development and completion of resident assessment and care plans in compliance with federal, state and local guidelines (MDS/RAI).
• Schedule OBRA and Medicare/managed care assessments, entry/re-entry records, and care plan reviews.
• Inform residents and families of Medicare/managed care coverage status and coordinate with CHC case managers.
• Participate in committees as directed.
• This role is on-call one weekend per month.
