American Senior Communities is seeking an experienced MDS Coordinator at Bethlehem Woods to oversee the Resident Assessment Instrument (RAI) and interdisciplinary care planning, ensuring compliance with state and federal regulations. The RN will coordinate the MDS process, mentor staff, and support ongoing quality improvement.

The role emphasizes clinical judgment, collaboration, and compassionate resident care within a respected Indiana-based senior care provider.

Qualifications

• Graduate of an accredited school of nursing, preferably BSN.
• Minimum of one year in nursing management in the long-term industry.
• Two years of professional nursing experience in long-term care, acute care, restorative care or geriatric nursing setting.

Responsibilities

• Coordinate and complete the Resident Assessment Instrument (RAI) and the interdisciplinary care planning process.
• Ensure compliance with state and federal regulatory requirements.
• Demonstrate CAR E values in resident, family, and staff interactions.
