Job summary

American Senior Communities is seeking an MDS Coordinator for Bethlehem Woods in Indiana. The RN will coordinate the Resident Assessment Instrument (RAI) and lead the interdisciplinary care planning process to ensure regulatory compliance and high-quality resident care.

Ideal candidates will have BSN preferred, with experience in nursing management in long-term care and strong CARE values guiding daily duties and team collaboration. This role offers full-time hours and comprehensive benefits.

Qualifications

• Graduate of an accredited school of nursing (BSN preferred).
• Minimum of one year in nursing management in long-term care.
• Two years of professional nursing experience in long-term care, acute care, restorative care or geriatric settings.

Responsibilities

• Coordinate and ensure completion of the Resident Assessment Instrument (RAI).
• Lead interdisciplinary care planning aligned with regulatory requirements.
