Job summary

Majestic Care of Manchin is seeking an MDS Coordinator (RN) to oversee MDS and care plans for residents in a timely, compliant manner within a long-term care facility.

Requires an active RN or LPN license, familiarity with MDS 3.0, and experience in long-term care. The role emphasizes collaboration, timely PPS submissions, and protecting resident rights while coordinating care team efforts.

Qualifications

• Must hold current RN or LPN license in the state of employment.
• Experience in Long Term Care preferred.
• Computer skills such as Data Entry and Word processing required.
• Must be knowledgeable of nursing practices and regulations for skilled facilities.
• Must possess CPR license or ability to obtain within 30 days.

Responsibilities

• Coordinate and supervise the MDS and Care Plan as outlined by facility policies.
• Inform care team of due dates and ensure completion of MDS portions.
• Develop or revise resident care plans quarterly or with significant changes.
• Coordinate PPS and state MDS assessments; ensure MDS, CATs, CAAS are completed.
• Submit MDS to state/federal agencies per Medicare/Medicaid guidelines and maintain timelines.
• Maintain resident rights, confidentiality, and inter-department communication.
• Assist with pre-admission screening to estimate RUGs when needed.
