Job summary

Edenbrook Edina, a trusted member of the Eden Senior Care Network, seeks a compassionate MDS Coordinator to join our skilled nursing facility in Edina, MN. The ideal candidate has MDS/RAI experience, strong ICD-10 knowledge, and familiarity with Point Click Care (PCC).

This role involves coordinating MDS, care plans, and communications with families and insurers. We offer a supportive environment, with competitive benefits and a focus on quality outcomes.

Qualifications

• Current license as LPN or RN in Minnesota (RN preferred).
• 3+ years’ experience in MDS/RAI.
• Strong knowledge of ICD-10 coding and Medicare/Rai guidelines.
• MDS process knowledge and PDPM familiarity.
• Proficient with Excel, Teams, and Zoom.

Responsibilities

• MDS scheduling, setup and completion.
• New admission authorizations and communications with families/insurance providers.
• ICD-10 coding for new admissions and LTC population updates.
• Support care plan development and updates.
• Attend family/resident care conferences and discharge planning.
• Educate IDT on MDS needs and service requirements.
• Review PDPM scores and ensure documentation accuracy.
• Maintain quality measures and participate in QAPI processes.
• Assist during surveys and chart audits.
