Job summary

Eddy Heritage House in Troy, NY is seeking an experienced MDS Coordinator to manage the Resident Assessment Instrument (RAI) process and ensure timely, accurate MDS assessments for optimal Medicare/Medicaid reimbursement.

You will collaborate with interdisciplinary teams, lead care plan meetings, and maintain regulatory compliance. Requires NY RN license, ADN; MDS certification preferred, BSN and RAC-CT beneficial.

Qualifications

• RN license in New York and current nursing license.
• Associate's Degree in Nursing required.
• MDS Certification or willingness to complete within timeframe.
• Knowledge of MDS, OBRA, PDPM, and reimbursement guidelines.
• Excellent communication and collaboration skills.
• Strong attention to detail and ability to meet deadlines.

Responsibilities

• Complete and submit all MDS assessments accurately and within federal and state timelines.
• Coordinate Medicare PDPM, PPS, and OBRA assessment schedules.
• Ensure documentation reflects services provided and supports reimbursement requirements.
• Perform electronic submission of MDS data to the State of New York.
• Prepare Care Area Assessments (CAAs) and collaborate with nurse managers for care plan development.
• Lead and participate in care plan meetings and ensure plans meet clinical and regulatory standards.
• Monitor all assessments and care plans for completeness, accuracy, and compliance.
• Oversee Medicare-related documentation from admission through discharge.
• Participate in determining Medicare Part A eligibility and support PDPM reimbursement strategies.
• Serve as an MDS/PPS resource for nursing, therapy, and administrative teams.
• Collaborate with the Schuyler Ridge MDS Specialist and the SPHP MDS Team.
• Support communication across all clinical departments to ensure coordinated resident care.
