Job summary

Edenbrook Second Avenue, part of the Eden East Network, seeks an experienced MDS Coordinator to manage MDS/RAI activities, coding, and reimbursement processes. The role involves coordinating with families, case managers, and insurers to ensure accurate PDPM outcomes and ongoing care planning.

Candidates should hold an LPN or RN license in PA (RN preferred) with 3+ years in MDS-related duties, and proficiency with PCC, Excel, Teams, and Zoom.

Qualifications

• Current license as LPN or RN in Pennsylvania; RN preferred.
• Strong knowledge of the MDS process and RAI.
• 3+ years' experience preferred in MDS/reimbursement.
• Familiarity with ICD-10 coding, Medicare Manual guidance, RAI Manual.
• Excellent written and oral communication; highly organized.

Responsibilities

• Support clinical reimbursement processes and lead RNAC in the facility.
• Schedule and complete MDS activities; ensure timely updates.
• Handle new admission authorizations and communications with families/insurers.
• Code ICD-10 for new admissions and update for LTC population.
• Participate in resident care plan development and reviews.
• Educate IDT on MDS needs and PDPM scoring; run utilization meetings.
