Job summary

Canton Center for Nursing and Healing is seeking an MDS Coordinator to manage and coordinate CMS-aligned MDS assessments in a fast-paced long-term care setting. You will work with nursing and therapy teams to ensure timely, accurate documentation and improve care quality across the facility.

Qualifications

• Strong organizational and communication skills.
• Knowledge of CMS guidelines and care planning.
• Long-term care experience preferred.
• RN license (GA) required.
• Experience with MDS/RAI processes is a plus.

Responsibilities

• Coordinate and oversee all MDS assessments per CMS and LTCI guidelines.
• Collaborate with nurses, therapists, and department heads for accurate documentation.
• Prepare and analyze QM/QI data for performance improvement.
• Ensure compliance with all RAI and MDS procedures and deadlines.
