Job summary

Genesis is seeking an experienced Registered Nurse in Warwick, RI to support long-term care residents through MDS coordination and care planning. You will ensure proper reimbursement, participate in assessments, and help develop individualized care plans in collaboration with the interdisciplinary team.

The role requires prior MDS/Care Plan experience, knowledge of long-term care regulations, and ongoing professional development to maintain compliance and high-quality patient care.

Qualifications

• Must be a Registered Nurse licensed by the state and in good standing.
• Experience in hospital, long-term facility, or healthcare setting required.
• Experience completing MDSs and Care Plans required.
• Knowledge of general and long-term care practices, regulations and guidelines.

Responsibilities

• Ensure appropriate reimbursement for patient care services.
• Participate in the Patient Assessment Process through MDS and CAAs.
• Develop individualized patient care plans and identify needs and outcomes.
• Contribute to the RAI process including MDS encoding and transmission.
• Identify changes in patient condition and coordinate assessments.
• Assist with Activities of Daily Living training for new hires and ongoing education.
• Attend educational programs to stay current with RAI/Prospective Payment System regulations.
