Job summary

Life Care Centers of America in Auburn, MA seeks an RN MDS Coordinator to lead accurate and timely interdisciplinary MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual regulations.

Requires RN license, associate or bachelor's degree in nursing, 2 years of nursing experience with geriatric care preferred; CPR certification; CRNC certification; strong knowledge of post-acute care regulations and a commitment to quality patient care.

Qualifications

Requires RN license in good standing and an active nursing license.
Two (2) years' nursing experience, geriatric nursing preferred.
CPR certification upon hire or during orientation; must remain current.

Responsibilities

Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations.
Report changes in a patient's condition identified by the MDS Assessment to the DON.
Provide education to direct care staff regarding CMS RAI Manual updates or Skilled Nursing Facility regulations.
Assist with review of the Interdisciplinary Comprehensive Care Plan.
Review Final Validation Reports and attest that assessments are completed and accepted into CMS QIES prior to billing.
