Qualifications

Responsibilities

• Coordinate MDS Assessments, CCAs, and Care Plans.
• Report changes in patient condition to the DON.
• Educate staff on CMS RAI Manual updates impacting documentation.
• Review Interdisciplinary Care Plans and Final Validation Reports.
• Ensure CMS QIES submissions are timely and accurate.
