Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to oversee and coordinate MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI regulations. The role requires an active RN license, two years of nursing experience (geriatric preferred), and CPR certification either at hire or during orientation.

You will liaise with an interdisciplinary team, ensure accurate documentation, and monitor verification of CMS reports and billing readiness.

Qualifications

• Must hold an active RN license in good standing.
• Two years of nursing experience; geriatric experience preferred.
• CPR certification on hire or during orientation.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans per CMS RAI Manual regulations.
• Report changes in patient condition identified by MDS to the DON.
• Educate direct care staff on CMS RAI Manual updates affecting documentation.
• Assist with review of the Interdisciplinary Comprehensive Care Plan.
• Review Final Validation Reports and ensure CMS QIES submissions prior to billing.
