Job summary

FutureCare in Baltimore, MD is seeking an RN Assessment Coordinator/MDS to lead the development and accurate completion of the MDS in line with RAI processes and federal/state regulations. You will collaborate with the IDT, monitor clinical documentation, and ensure timely MDS scheduling and coding accuracy using MDS software.

The role requires active Maryland RN licensure with 3+ years’ MDS coordination experience, PDPM knowledge, and strong interpersonal skills.

Qualifications

• Active RN licensure to work in Maryland.
• 3+ years’ experience as an MDS-Registered Nurse Assessment Coordinator with ICD-10 coding.
• Progressive nursing experience in a resident assessment environment.
• Managerial and interpersonal skills.
• Proficiency in PDPM and clinical documentation requirements.
• Ability to build positive relationships with an interdisciplinary team.
• Willingness to learn and teach, with adaptability to change.
• In-depth PDPM/CMI knowledge related to Medicare/Medicaid reimbursements.

Responsibilities

• Oversee and coordinate the development and accurate completion of the MDS according to current RAI processes, Federal and state regulations.
• Collaborate with the Interdisciplinary Team and monitor clinical documentation for timely MDS scheduling and completion.
• Partner with IDT to evaluate Quality Measures.
• Review completed MDS assessments for accuracy, including using MDS software to address coding inconsistencies prior to locking.
