Job summary

Qualifications

• Active nursing license in the relevant state.
• Experience administering medications per physician orders.
• Proficiency with PCC electronic medical records.
• Knowledge of memory care protocols and MDS assessments.
• Experience in long-term/sub-acute care or dementia units.
• Strong communication with residents, families, and team.

Responsibilities

• Assess residents' health problems and develop care plans.
• Administer medications and treatments as prescribed.
• Monitor progress and respond to changes in condition.
• Collaborate with multidisciplinary teams for care plans.
• Educate residents and families on conditions and care.
• Maintain HIPAA-compliant, accurate records.
• Support case management and continuity of care.
• Participate in specialized areas such as PT/OT/SUD/pain management.
