Qualifications

• Licensed RN or LPN in Massachusetts.
• Proficiency with medication administration.
• Experience with memory care and MDS assessments.

Responsibilities

• Assess resident health problems and develop nursing care plans.
• Administer medications and treatments as prescribed.
• Monitor resident progress and respond to changes in condition.
• Collaborate with multidisciplinary teams for comprehensive care.
• Educate residents and families on health conditions and discharge.
• Maintain accurate records in compliance with regulations and HIPAA.
• Participate in case management to ensure continuity of care.
• Support in specialized areas (PT, OT, SUD, Pain management).
