Royal Fairhaven Nursing and Rehabilitation Center is seeking an RN/LPN to provide compassionate resident-centered care in a skilled nursing environment.

You will administer medications, develop care plans, monitor progress, and collaborate with a multidisciplinary team across long-term, sub-acute, and memory care units. A $5,000 sign-on bonus, weekly pay, tuition reimbursement, and EAP support are offered to eligible applicants.

Qualificações

• Active RN or LPN license in the state.
• Experience in long-term and memory care environments.
• Proficient in administering medications and using EMR systems.
• Strong communication with residents, families and healthcare teams.

Responsabilidades

• Assess resident health problems and develop nursing care plans.
• Administer medications and treatments as prescribed.
• Monitor progress and respond to changes in condition.
• Collaborate with multidisciplinary teams for comprehensive care.
• Educate residents and families on health conditions and discharge.
• Maintain accurate records in compliance with HIPAA and regulations.
• Participate in case management for continuity of care.
• Support in specialized areas such as PT, OT, SUD, Pain management.
