Job summary

Dublin Post Acute in Dublin, OH is seeking a compassionate RN/LPN to join our day shift team. The RN/LPN will provide high‑quality nursing care, supervise clinical staff, and ensure residents' health and safety in a skilled nursing and rehabilitation setting.

Responsibilities include assessing and updating care plans, administering medications, timely documentation, and collaborating with physicians and the interdisciplinary team.

Qualifications

• Current, active RN license in good standing.
• Graduate of an accredited nursing program.
• CPR/BLS certification required.
• Experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred.

Responsibilities

• Assess, plan, implement, and evaluate resident care plans.
• Administer medications and treatments as prescribed.
• Monitor residents' conditions and report changes promptly.
• Supervise and support LVNs, CNAs, and other nursing staff.
• Document accurately per facility policy and regulatory requirements.
• Participate in shift reports, care conferences, and quality improvement activities.
• Promote a positive, resident-centered environment focused on quality care and customer service.
