Responsibilities include administering meds, documenting care, and supporting admissions and discharges. A strong team player with reliable attendance is essential to deliver high-quality post-acute care.

Qualifications

• Current Ohio LPN license in good standing.
• Previous long-term care or skilled nursing experience preferred.

Responsibilities

• Provide direct nursing care to residents in accordance with their care plans.
• Administer medications and treatments as ordered.

• Assist with admissions, transfers, and discharges as needed.
• Communicate effectively with residents, families, physicians, and the healthcare team.
