Job summary

Mayo Clinic is seeking a Transplant Coordinator Nurse to oversee care coordination for the transplant patient population. You will collaborate with patients, families and a multidisciplinary team to plan and execute care across the continuum.

Required is an RN with a BSN or ACEN/CCNE-accredited program, state licensure, and at least 1 year of clinical RN experience (3 years preferred). This on-site role in Minnesota offers TN sponsorship eligibility and a path for advancement.

Qualifications

• Graduate of a BSN or ACEN/CCNE-accredited nursing program.
• RN license by applicable state requirements.
• Minimum 1 year of clinical RN experience; 3 years preferred.
• Strong leadership, interpersonal and communication skills; adaptable and autonomous.

Responsibilities

• Coordinate all aspects of care for transplant patients with patient, caregiver and multidisciplinary team.
• Assess data and plan care to meet age-specific needs across the care continuum.
• Provide patient and family education on an ongoing basis.
• Demonstrate leadership through quality improvement, research and committee work.
