Mayo Clinic in Minnesota seeks a Transplant Coordinator Nurse to coordinate all aspects of transplant patient care in coordination with patients, families and a multidisciplinary team.

You will assess patient data, plan care to meet age-specific needs, and provide ongoing education to patients and families. The role requires RN licensure and flexibility in hours, with opportunities for leadership and involvement in quality improvement initiatives.

Qualifications

• Graduate of a ACEN or CCNE-accredited nursing program.
• Minimum 1 year of clinical RN experience; 3 years preferred.
• RN license by applicable state requirements.

Responsabilités

• Coordinate all aspects of care for the transplant patient population in collaboration with patient, caregiver and multidisciplinary team.
• Plan and assess data to meet age-specific needs; educate patients and families consistently.
• Provide leadership through quality improvement, committee work and research; adhere to ANA/UNOS/FACT guidelines.
