Job summary

Mayo Clinic in Minnesota is seeking a Transplant Coordinator Nurse to coordinate all aspects of care for the transplant patient population. You will assess needs with patients, families, and the multidisciplinary team, plan care, and educate patients consistently.

This role requires RN licensure in the applicable state, a BSN from an ACEN/CCNE accredited program, and a minimum of 1 year RN experience (3 years preferred).

Qualifications

• RN license by applicable state requirements.
• Graduate of a BSN or ACEN/CCNE accredited nursing program.
• Minimum 1 year of clinical RN experience; 3 years preferred.
• Ability to work flexibly in a patient care setting and lead effectively.

Responsibilities

• Coordinate all aspects of care for the transplant patient population with the care team.
• Assess and plan in collaboration with patient, caregiver, and multidisciplinary team.
• Provide patient and family education on a consistent basis.
