Job summary

Under the supervision of the Nurse Supervisor, you will assess patients, develop care plans, administer medications, and collaborate with an interdisciplinary team. This role emphasizes teamwork, leadership, and ongoing professional growth through education, tuition support, and specialty certifications.

Qualifications

• Graduate of an accredited School of Nursing (ADN or BSN)
• Active RN license in the state
• Current BLS/CPR certification
• Clinical experience in long-term or geriatric care preferred
• English proficiency for documentation and communication
• Health screenings as required by state guidelines

Responsibilities

• Perform comprehensive clinical assessments and administer medications/treatments
• Develop individualized care plans to optimize recovery and reduce readmissions
• Coordinate with interdisciplinary teams and physicians for patient care
• Supervise and delegate tasks to LPNs and CNAs
• Maintain accurate real-time documentation using PCC EHR
• Ensure compliance with HIPAA and safety standards
