Job summary

ChenMed is seeking an Intensive Community Manager (RN) to lead complex care for high-risk patients. You will perform field nursing, develop comprehensive care plans, and supervise team members to keep patients safely at home. Engage with PCPs to tailor plans and ensure quality care.

This role requires RN licensure, at least 2 years of clinical experience, and strong organizational skills. Travel up to 10% and a focus on patient-centered outcomes are expected.

Qualifications

• Associates degree in Nursing is required.
• Registered Nurse license in State of employment is required.
• BSN or higher preferred.
• Minimum of 2 years of clinical experience.
• Experience in case management is desirable.

Responsibilities

• Provide in-house, at facility, and telephonic visits to high-risk patients with goal of preventing unnecessary hospital arrivals.
• Perform home visits for field nursing interventions and care plan development.
• Supervise LPNs and oversee patient discharge from case management.
• Conduct SDoH, disease monitoring, and health education assessments in outpatient/home settings.
• Coordinate care plans and monitor patient progress with PCPs and care team.
