Qualifications

• Active RN license in state of employment.
• Minimum five years clinical work experience.
• Experience in case management or discharge planning preferred.
• BLS certification within first 90 days of employment.

Responsibilities

• Manages a team of ICCMs to reduce hospital arrivals.
• Oversees recruitment, performance management, scheduling, and staffing.
• Ensures CMP, CM, and DM plans are consistently executed.
• Collaborates with PCPs, specialists, and care teams to optimize patient care.
• Promotes high-quality patient care and efficient resource use.
• Travel locally, regionally, and nationwide up to 30%.
