Job summary

ChenMed is seeking an RN Case Manager (Intensive Community Manager) to lead care for our high-risk patients across in-office facilities in the Richmond region and at-home visits. You will design comprehensive care plans, supervise team members, and collaborate with PCPs to keep patients safely at home and avoid hospitalizations.

Qualifications include an RN license and 2+ years of clinical or case management experience; BSN preferred.

Qualifications

• Associate degree in Nursing required.
• BSN or RN with a related bachelor’s degree preferred.
• Current RN license in state of employment; compact license where available.

Responsibilities

• Provide in-house, facility, and telephonic visits to high-risk patients to prevent hospital admissions.
• Perform home visits and develop care plans addressing goals and barriers.
• Supervise LPNs and oversee patient discharge from case management.
• Conduct health assessments, fall risk, and social determinants of health (SDoH).
• Coordinate plan of care with PCPs and care team; engage families and community resources.
