Job summary

ChenMed LLC is seeking a Manager, Intensive Community Complex Care (ICC) to lead regional ICCM teams, drive care coordination, and optimize patient outcomes across hospital and community settings.

The role focuses on proactive case management, CMP/DM plans, and collaboration with PCPs and specialists to prevent unnecessary hospital use while maintaining high-quality patient care. A RN license and nursing leadership experience are required.

Qualifications

• Active RN license in the state of employment.
• BSN or ADN with 2+ years of home health, case management or discharge planning experience.
• BLS certification within first 90 days of employment.
• Case management experience preferred.

Responsibilities

• Lead ICCM team to reduce hospital admissions and manage patient load.
• Oversee CMP, CM and DM plans and ensure adherence.
• Coordinate care with PCPs, specialists, and ancillary teams.
• Mentor ICCMs and provide ongoing education and coaching.
• Manage staffing, recruitment, and performance of team.
