Job summary

ChenMed is seeking an Intensive Community Manager (ICCM), an RN who leads a Complex Care Team to support high-risk patients through in-home visits and facility visits. The ICCM designs comprehensive care plans, coordinates with PCPs, and provides clinical supervision to team members while ensuring safe transitions and ongoing engagement in care.

Qualifications

• Associate degree in Nursing required.
• Bachelor’s Degree in Nursing (BSN) or RN with bachelor’s degree in related clinical field preferred.
• Active Registered Nurse license in state of employment required.

Responsibilities

• Provides in-house, at facility, and telephonic visits to high-risk patients to prevent unnecessary hospital admissions.
• Performs home visits and develops care plans addressing goals and interventions.
• Supervises and educates LPNs and other team members; coordinates plan of care with PCPs.
• Drives comprehensive plans of care and ensures patient safety and timely follow-up.
