Job summary

ChenMed is hiring an Acute Care Nurse to lead complex care for seniors. The role focuses on hospital-to-home transitions, discharge planning, and coordination with PCPs, hospital teams, and caregivers to ensure safe, timely care. You will work from clinics and hospital settings, coordinating with post-acute partners.

Requirements include a BSN or RN with related degree, active RN license, and 2+ years of clinical/ case management experience. Travel up to 10% may be needed.

Qualifications

• Bachelor’s Degree in nursing (BSN) or RN with related degree preferred.
• Active RN license in state of employment required.
• Compact License preferred where available.
• Minimum of two years clinical work experience.
• Experience in utilization review or case management is desired.

Responsibilities

• Be present at assigned hospitals during core hours and manage caseload.
• Identify opportunities for proper resource allocation in acute/post-acute settings.
• Assess safety, functional status, medications, and knowledge gaps.
• Collaborate with Complex Care and Clinical Strategy Teams on SOP execution.
• Explain Care Management/Disease Management programs to patients and families.
• Coordinate discharge planning and communication with PCPs and care teams.
• Support transitions to SNF/LTC and arrange post-discharge follow-up.
• Advise on level of care and participate in patient handoffs.
• Document discharge dates and update records.
• Screen Social Determinants of Health and refer as needed.
• Identify barriers to safe discharge and coordinate with specialists.
• Travel locally up to 10% as required.
