Job summary

ChenMed is seeking an Intensive Community Care Manager (RN) to lead a Complex Care Team serving our highest-risk patients. You will conduct in-home, facility, and telephonic visits, develop comprehensive care plans with PCPs, and supervise team members to prevent hospitalizations while keeping patients safely at home.

Ideal candidates hold an RN license with an associate degree, at least 2 years of clinical experience, and 1 year of community case management.

Qualifications

• Associate degree in Nursing is required.
• Active RN license in the state of employment.
• Minimum 2 years of clinical work experience.
• Minimum 1 year of community case management experience desired.
• Valid driver’s license required.
• BLS certification within 90 days of employment.

Responsibilities

• Provide in-house, facility, and telephonic visits to high-risk patients to prevent hospital admissions.
• Perform home visits to assess and update care plans with PCP collaboration.
• Supervise LPN and educate patients to ensure safe discharges.
• Conduct SdoH assessments and monitor health education and self-care instructions.
• Coordinate care plans with PCPs and care team to address patient needs.
