Job summary

CareSource seeks an Integrated Care Clinical Manager in Massachusetts to coordinate clinical care for dually-eligible enrollees with complex needs. The RN will engage in home visits, assess risks, and partner with the interdisciplinary team to develop comprehensive care plans.

The role emphasizes community integration, coordination of services, and education on Medicare/Medicaid benefits. Travel across the territory will be required to support patients’ outcomes.

Qualifications

• Requires an A.S. degree in nursing and RN license.
• Experience in care coordination or case management for dual-eligible populations preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage enrollees in home and community settings with culturally and linguistically appropriate care.
• Serve as liaison among providers, resources, and enrollees for seamless care transitions.
• Conduct comprehensive assessments and functional evaluations as needed.
• Develop and implement care plans with ICT and care teams.
• Oversee utilization of long-term supports to allow participant choice of living location.
