Job summary

CareSource in Massachusetts seeks an Integrated Care Clinical Manager to provide monitoring, follow-up, and clinical care management for dually-eligible enrollees with complex needs. You will coordinate health services and community resources to improve outcomes and quality of life.

The role involves leading the interdisciplinary care team, performing comprehensive assessments, and coordinating care transitions across providers to ensure seamless, person-centered care.

Qualifications

• Associates of Science in nursing from an accredited program.
• Current unrestricted MA RN license.
• Experience in care coordination or case management preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage with enrollees in homes to develop and monitor complex care plans.
• Coordinate care between healthcare providers, community resources, and enrollees.
• Perform comprehensive assessments (including Functional/Health assessments) and updates.
• Lead the interdisciplinary care team to create holistic care plans.
