Job summary

CareSource seeks an Integrated Care Clinical Manager in Massachusetts, a community-based RN responsible for monitoring, follow-up and clinical care management for dually-eligible enrollees with complex medical, behavioral, and social needs. Qualifications

• RN with ability to independently serve people with complex medical, behavioral, and social needs.
• Current MA license to practice as an RN required; case management certification preferred.
• Experience in care coordination or with dual-eligible populations preferred.

Responsibilities

• Engage enrollees in home and community settings to establish a complex care management relationship.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
• Coordinate with providers, community resources and state agencies to avoid service duplication.
• Educate members about benefits, services, preventive care and chronic condition management.
• Perform assessments such as Comprehensive Assessments and functional assessments.
