Qualifications

• Active Registered Nurse license.
• Ability to perform verbal and observational assessments.
• Strong documentation and care-planning skills.
• Comfortable working independently in the community.
• Professional, compassionate communication with clients, families, and caregivers.

Responsibilities

• Travel to clients' homes to complete scheduled non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and families.
• Review medical history, diagnoses, medications, and daily routines.
• Identify risks and support needs, and appropriate non-medical services.
• Develop and update individualized care plans for in-home caregivers.
• Serve as a clinical resource regarding the client’s plan of care.
• Maintain accurate and timely documentation of assessments and care plans.
