Job summary

ComForCare Home Care is a franchise of premier in-home care providers. As a Registered Nurse, you will conduct thorough client assessments, create personalized care plans, implement interventions to achieve client goals, update care plans to track progress, administer medications when necessary, and coordinate referrals to other healthcare professionals.

You will maintain detailed client records and travel between sites for visits, while upholding safety and infection control practices.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license in state of practice.
• CPR certification.
• Demonstrated knowledge of safety and infection control practices.
• Must have a reliable, insured personal vehicle and a valid driver’s license.

Responsibilities

• Perform comprehensive client assessments (physical, psychosocial, environmental).
• Develop individualized care plans incorporating data analysis and current findings.
• Provide interventions to achieve realistic client outcomes.
• Report changes to update the plan of care toward goals and outcomes.
• Administer medications as needed.
• Maintain detailed client records of assessments, planning, and evaluations.
• Initiate referrals to other health team members.
