Job summary

Qualifications

Responsibilities

• Travel to clients' homes to complete scheduled non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and family members.
• Review medical history, diagnoses, medications, and daily routines.
• Identify risks and needs for non-medical services.
• Develop and update individualized care plans for in-home caregivers.
• Serve as a clinical resource regarding the client's plan of care.
• Maintain accurate documentation of assessments and care plans.
