Job summary

Qualifications

• Active RN license and ability to perform thorough verbal and observational assessments.
• Strong documentation and care-planning skills.
• Comfortable working independently in the community.

Responsibilities

• Travel to clients’ homes to complete scheduled, non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and family members.
• Review medical history, diagnoses, medications, functional abilities, and routines.
• Identify risks and appropriate non-medical services, develop care plans.
• Serve as a clinical resource to the care team and maintain documentation.
