Job summary

ComForCare is seeking a Registered Nurse to perform in-home, non-clinical assessments and care planning without direct bedside care. The role focuses on conversation, observation, documentation, and coordinating non-medical services for clients and families.

Travel to clients’ homes is required for scheduled visits. You will review health history, develop practical care plans for in-home caregivers, and serve as a clinical resource to the care team while maintaining thorough records.

Qualifications

• Active Registered Nurse license.
• Ability to complete thorough verbal and observational assessments.
• Strong documentation and care-planning skills.
• Comfortable working independently in the community.

Responsibilities

• Travel to clients’ homes to complete scheduled, non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and family members.
• Review medical history, diagnoses, medications, functional abilities, and daily routines.
• Identify risks, support needs, and appropriate non-medical services.
• Develop and update individualized care plans for in-home caregivers to follow.
• Serve as a clinical resource to the care team regarding the client’s plan of care.
• Maintain accurate and timely documentation of assessments and care plans.
