Job summary

ComForCare Home Care - Oklahoma City is seeking an Registered Nurse to perform in-home visits focused on assessment, planning, and caregiver guidance. This role emphasizes conversation, observation, and care coordination without direct hands-on patient care.

The RN will travel to clients’ homes to conduct non-clinical nursing assessments, review health history, and develop practical care plans for in-home caregivers, serving as a resource for the care team.

Qualifications

• Active RN license in good standing.
• Ability to conduct thorough verbal and observational assessments.
• Strong documentation and care-planning skills.
• Capable of independent work in community settings.
• Professional, compassionate communication with clients and families.

Responsibilities

• Travel to clients’ homes to complete scheduled, non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and family members.
• Review health history, diagnoses, medications, functional abilities, and daily routines.
• Identify risks, support needs, and appropriate non-medical services.
• Develop and update individualized care plans for in-home caregivers to follow.
• Serve as a clinical resource to the care team regarding the client’s plan of care.
• Maintain accurate and timely documentation of assessments and care plans.
