Job summary

ComForCare Home Health Care - Tulsa is seeking a Registered Nurse to perform non-clinical in-home assessments and develop practical care plans for in-home caregivers. This role emphasizes conversation, observation, and documentation over bedside care, with travel to clients’ homes for scheduled visits.

You will review health history, medications, and daily routines, identify risks, and serve as a resource to the care team.

Qualifications

• Active RN license required.
• Ability to conduct thorough verbal and observational assessments.
• Strong documentation and care-planning skills.
• Comfortable working independently in the community.

Responsibilities

• Travel to clients’ homes to complete scheduled, non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and family members.
• Review medical history, diagnoses, medications, functional abilities, and daily routines.
• Identify risks, support needs, and appropriate non-medical services.
• Develop and update individualized care plans for in-home caregivers to follow.
• Serve as a clinical resource to the care team regarding the client’s plan of care.
• Maintain accurate and timely documentation of assessments and care plans.
