Responsibilities

• Develop individualized care plans including discharge planning.
• Administer direct patient care and medications.
• Practice infection control and maintain a safe environment.
• Communicate changes in patient condition to the Charge RN/Clinician.
• Maintain and update written care plans and patient education.
