Job summary

MaineHealth CHANS Home Health & Hospice seeks a Clinical Coordinator to lead day-to-day hospice and home-health patient care coordination. You will ensure effective operations meeting patient needs and guide the clinical team to deliver reliable, evidence-based, patient-family centered care.

The role requires a Bachelor’s in Nursing or advanced clinical degree, RN/PT/OT licensure with a valid driver’s license, and at least three years in home health or hospice.

Qualifications

• Education: Bachelor’s Degree in Nursing or other advanced clinical degree required.
• License/Certifications: Appropriate clinical license (RN, PT, or OT) required. Valid Driver’s License required.
• Experience: Three years in home health and/or hospice experience required.

Responsibilities

• Coordinate day-to-day hospice/home-health patient care services and ensure effective operations of patient care needs.
• Collaborate with the clinical team to guide staff and deliver high-quality, patient-family centered care.
