Freedom Home Health and Hospice Care Services Inc. is seeking a Clinical Supervisor to ensure coordinated and managed patient care and to supervise clinical personnel.

The role focuses on directing case referrals, assigning hospice staff, and collaborating with the Medical Director to determine eligibility for services. The position requires current RN licensure, BSN preferred or equivalent nursing experience, and至少 two years of supervisory experience in hospice/home care settings.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• BSN degree preferred or at least one year of professional nursing experience within the last three years.
• Hospice/home care experience and two years of management or supervisory experience.
• Complies with accepted professional standards and practice.
• Excellent observation, nursing judgment and communication skills; understands hospice philosophy and death/dying.
• Understands pain and symptom management.
• Must be a licensed driver with insured vehicle and good working order.
• Maintains current CPR certification.
• Strong written and verbal communication skills.

Responsabilités

• Receives case referrals and assigns hospice personnel; coordinates care.
• Reviews cases via home visits, conferences, record reviews and clinician services; guides performance.
• Assists in establishing short- and long-term goals and care plans.
• Participates in case conferences to coordinate care and interdisciplinary input.
• Conducts quarterly record reviews and shares findings with leadership.
• Assists in screening and interviewing new employees and orientations.
• Supports policy development, education programs and quality initiatives.
• Engages in community relations to promote home health services.
• Performs other duties as delegated by Clinical Director.
