Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Registered Nurse to deliver rapid hospice field visits across Palm Beach County. The role features a 7pm–7:30am shift with every other weekend, providing urgent and emergent in-home care with autonomy and collaboration with physicians and the interdisciplinary team.

You will triage, assess, intervene, educate, and coordinate care with primary physicians to prevent unnecessary ER visits.

Qualifications

• Florida RN license required to practice in Florida.
• BLS certification is required/co-approved.
• One year in public health, home health, oncology, med-surg, geriatric or hospice nursing preferred.

Responsibilities

• Provide rapid-response, in-home hospice nursing care in Palm Beach County.
• Assess, triage, and manage urgent and emergent health conditions outside clinics.
• Collaborate with physicians and interdisciplinary team to coordinate care.
• Monitor patient conditions, deliver interventions, and provide education to patients and families.
