Job summary

Trustbridge Hospice, part of Empath Health, seeks an RN for our Gerstenberg Inpatient Care Center in West Palm Beach. You will coordinate end-of-life care, assess patient and family needs, and educate on comfort and quality of life. Weekend rotations and a 12-hour shift are included.

Qualifications

• Current Registered Nurse license in Florida.
• BLS Certification on active status.
• 1 year in hospice, public health, home health, oncology, or geriatric nursing preferred.

Responsibilities

• Coordinate patient and family care with primary physician and IDG.
• Perform comprehensive assessments of patient and family needs.
• Ensure timely delivery of interventions per care plan.
• Apply clinical expertise to monitor patient conditions and educate on comfort and quality of life.
• Address concerns to uphold satisfaction and trust.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to join our Gerstenberg Inpatient Care Center team in West Palm Beach. What You’ll Do

As an RN in our inpatient care center, you will play a vital role in coordinating and delivering compassionate end‑of‑life care:

• Coordinate patient and family care in collaboration with the primary physician and interdisciplinary team (IDG)
• Conduct thorough assessments of patient and family needs
• Ensure timely and coordinated delivery of interventions and services outlined in the care plan
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
