Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Hospice Registered Nurse Case Manager in Palm Beach County. The role requires an active Florida RN license, BLS certification, and provides patient-centered care while guiding families through the end-of-life journey; schedule includes Monday–Friday 8:30am–5:00pm with a weekend requirement.

Qualifications

• Active Florida RN license.
• Current BLS certification.
• 1 year in public health, home nursing, oncology, medical/surgical, geriatric or hospice nursing preferred.
• Proficiency with EMR systems and basic computer skills.

Responsibilities

• Advocate for patients and families, ensuring needs are addressed.
• Coordinate care under physician direction with the IDG.
• Develop and implement individualized plans of care.
• Monitor patient conditions and deliver education to improve comfort and quality of life.
• Address concerns promptly to maintain satisfaction.
